Reconstructive Surgery: what it involves

Reconstructive orthopaedic surgery rebuilds limbs that have been damaged rather than simply broken. It covers bone loss after severe trauma or infection, limbs that have healed short or crooked, congenital and developmental deformities such as bow legs and knock knees, and joints that need to be preserved rather than replaced in a young patient.

Recovery

Reconstruction is measured in months rather than weeks. Gradual lengthening or bone transport typically runs for a correction phase followed by a longer consolidation phase, and physiotherapy runs throughout to keep the neighbouring joints supple and the muscles working.

How much can a limb be lengthened?
Lengthening is done gradually, at roughly one millimetre per day, and the safe amount depends on the bone, the soft tissues and the reason for the shortening. Several centimetres are commonly achievable in a single stage, and the target is set individually after assessment.
Is an external fixator painful to live with?
A frame is uncomfortable in the first weeks and then becomes manageable. Pin sites need daily cleaning, which patients are shown how to do themselves. Most people walk on the frame and continue much of their routine while treatment is under way.
Can bow legs or knock knees be corrected in adults?
Yes. In adults, angular deformity around the knee is corrected with an osteotomy, which realigns the limb and shifts load off the worn side of the joint. In a younger patient this can delay or avoid the need for a knee replacement.
Can chronic bone infection be cured?
Chronic osteomyelitis is treatable, but it takes staged surgery: thorough removal of dead and infected bone, local and systemic antibiotics, and then reconstruction of the resulting defect once the infection is controlled. The plan and its timeline are explained before starting.
